The UK Turner syndrome (TS) study examined the effect on final height of oxandrolone 0.05 mg/kg/day (maximum dose 2.5 mg) versus placebo from 9 years of age; and delaying ethinylestradiol induction of puberty by 2 years from 12 (E12) to 14 (E14) years in growth hormone-treated girls with TS. The study ran from 1999 to 2013. By 2011, eighty-two of 92 participants had reached final height and an interim analysis using the Super-Imposition by Translation And Rotation model showed significant increases in final height with both oxandrolone and E14. The analysis has been repeated now that all 92 patients have reached final height. Oxandrolone still significantly increased final height by 4.1 cm (95% CI 1.6 to 6.6, n=92) compared with 4.6 cm previously. The epidemiology of meningococcal disease is changing in the UK. It is now a rare cause of non-blanching rashes in children presenting to the emergency department. All paediatricians will have been faced with a child with a non-blanching rash. The dilemma is well rehearsed: should this child have invasive investigations and administration of intravenous antibiotics in order to treat possible meningococcal disease or should they simply be observed for some time or indeed discharged with a clinical diagnosis of probable enterovirus without any intervention. The risk is of course, missing severe sepsis and the possible unnecessary death of a child vs inflicting many more children to unnecessary investigations, over treatment with antibiotics and unnecessary hospital admission and cost and contributing to the more global issue of antimicrobial resistance. Accommodation outcomes for children of drug-misusing women

Children of drug-misusing women are a vulnerable group. Data describing the pattern of accommodation placements are lacking. We investigated 10- to 12-year accommodation outcomes of children born to drug-misusing mothers at a single maternity hospital. 94% of mothers were prescribed maintenance methadone during pregnancy and at least 87% poly-drug used. Data were successfully matched for 132 children (29% of the original cohort of 450 babies). These children had a total of 291 placements (median 2 (range 1–6)), only 28.5% of which were with the birth parents. At 10–12 years, 54 (41%) were in the care of their parent(s). 83% (109/132) were discharged from the maternity hospital to their birth parents; 41% of these children (54/132) remained with their parents at 10–12 years. Better understanding of the proportion of children who ever receive special educational needs (SEN) provision or social care services during school years is highly relevant for healthcare as reductions in one or more of these services could impact on healthcare. Using the National Pupil Database linked to the all-of-England children looked after return and children in need census, we estimated the cumulative incidence of SEN status among (1) children ever in care during school, (2) children in need but not care, and (3) neither. We observed a very high proportion of children who were in care or need during school years had SEN provision at some point (83% and 65%, respectively), and that a high proportion of children in neither of these groups did so, too (37%). Tablets are safer, more convenient and cheaper than liquid medications. Children and young people (CYP) often remain on liquids due to habit, reluctance to change or staff and parents’ lack of knowledge about switching to tablets. We describe a quality improvement project to train staff and embed a system of converting eligible children to tablet medication. A series of tests of change were made including training, making kit available, publicity and developing team protocols. In 3 months, 21 out of 25 eligible CYP were successfully converted with added benefit of saving £46 588 per year. Duration of initial antibiotic course is associated with recurrent relapse in protracted bacterial bronchitis

Protracted bacterial bronchitis (PBB) is the leading cause of chronic wet cough in young children from developed countries. Despite its high prevalence there is a paucity of evidence to inform the optimal duration of treatment leading to variation in practice. Relapse of chronic cough is common and recurrent PBB (>3 episodes in 12 months) is associated with a future diagnosis of bronchiectasis. We investigated the factors associated with any relapse (≥1 episode in 12 months) and recurrent PBB in 66 children. No factor was significantly associated with any relapse. Duration of initial antibiotic treatment was the only factor significantly associated with recurrent PBB. Those who received antibiotics for 6 weeks antibiotics were less likely to develop recurrent PBB than those who received for 2 weeks (p=0.046). This is the first study to show an association between duration of initial antibiotic course and therefore future bronchiectasis. Prospective studies are needed to investigate

Life-threatening hypersensitivity pneumonitis secondary to e-cigarettes

We report a case of hypersensitivity pneumonitis (HP) in a young person secondary to vaping. He presented with a putative diagnosis of asthma and required extracorporeal membrane oxygenation because of intractable respiratory failure. He developed a critical illness and steroid myopathy and required prolonged rehabilitation. Our patient fulfils diagnostic criteria for HP secondary to e-cigarettes with a positive exposure history, deterioration after skin prick testing, specific serum IgM antibodies against the implicated liquid raising the possibility that the relevant antigen was present in that liquid and radiological and histopathological features compatible with acute HP. There are two learning points. The first is always to consider a reaction to e-cigarettes in someone presenting with an atypical respiratory illness. Prescribing emergency oral steroids in asthma clinics

We retrospectively reviewed children who had been prescribed emergency oral corticosteroids (OCS) in a routine tertiary paediatric respiratory clinic appointment. We subsequently assessed adherence from prescription uptake of inhaled corticosteroids or combination inhalers in the 6 months prior to the episode. In 2 years, 25 children received 32 courses of prednisolone. Median adherence was 33%, but 28% for those with repeated OCS prescriptions. Prescribing acute OCS in a routine clinic is a red flag for potential poor adherence to preventer therapies, and may also indicate the child has poor perception of the severity of their symptoms. Intravenous immunoglobulin (IVIG) reduces coronary aneurysms in patients with Kawasaki disease (KD), but additional management options remain challenging, with no generalisable evidence-based recommendations. We performed a survey of 724 physicians from 73 countries to assess variation in practice. IVIG was the preferred initial treatment by 659 (91%) of respondents. Criteria for adjunctive primary treatment varied considerably and definitions of IVIG resistance varied markedly by geographical continent, Human Development Index tiers and medical specialty. A second dose of IVIG was used most often for patients with coronary aneurysm non-responsive to initial treatment (572, 79%), but corticosteroids (379, 52%) and tumour necrosis factor alpha inhibitors (208, 29%) were also frequently used. It requires a large multi disciplinary team at the patient and family level and major public health and political drivers at another. There is a compendium of drugs that have been studied as potential enhancers of weight loss. The Archivist has never really been impressed by the quick fix concept of a tablet for weight loss. Optimising attendance rates in outpatient’s clinics is important to ensure good clinical care and to avoid waste of scarce medical resources. We identified a cohort of teenagers that frequently were not brought (WNB) to the paediatric diabetes multidisciplinary clinics, compromising patient care and reducing compliance with National Padiatric Diabetes Audit (NPDA) standards. As these teenagers attend five local secondary schools, we initiated a quality improvement project to test the benefit of taking clinics into schools. From January 2017, after extensive engagement with stakeholders, quarterly clinic at school with a diabetic nurse and physician were offered to all teenagers with type 1 diabetes mellitus in these five schools. A semistructured interview was administered to identify their concerns about diabetes. HbA1c was checked, and glucose meters were downloaded. Wilson disease (WD) is a rare, recessively inherited disorder of copper metabolism mainly affecting liver and brain. In childhood, it is known to have a predominant hepatic phenotype. It is likely that the low awareness for WD-associated neuropsychiatric signs and symptoms in this age group means that neurological Wilson’s disease is underdiagnosed in children and young people. Practitioners should be alert for this complication in children with or without liver disease. Management of children with WD requires a dedicated multidisciplinary approach involving hepatologists, geneticists, neurologists and psychiatrists to ensure subtle neuropsychiatric symptoms are identified early and addressed appropriately. What does 'PEWing or 'PEWsing mean? A new word in paediatrics

Since 2015, a national Paediatric Early Warning System (PEWS) has been used in acute paediatric settings in the Ireland. At a national PEWS workshop, the terms ‘pewing’ and ‘pewsing’ were noted to have been used or overheard in clinical conversations in some settings. We sought to understand the term ‘pewing’ or ‘pewsing’ interviewing 40 frontline paediatric nurses and doctors in a tertiary hospital and 20 from other regional sites. We asked, ‘what does the term ‘pewing’ or ‘pewsing’ mean to you?’ 95% (38/40) of staff asked at Children Hospital Ireland at Tallaght Hospital heard or used the term daily. No responses indicated the same understanding of the term.
