Recommendations by the European Network of Paediatric Research at the European Medicines Agency (Enpr-EMA) Working Group on preparedness of clinical trials about paediatric medicines process

Conduct of clinical trials in babies, children and young people is often hindered by issues that could have been foreseen before the trial opened; that is, some clinical trials are often underprepared. In order to identify a good approach to trial preparedness, the European Network of Paediatric Research at the European Medicines Agency formed a working group. The Working Group included representation from regulators, industry, academics, paediatric clinical research networks and parents. The Working Group consulted widely about how to prepare for paediatric clinical trials. The Group’s detailed recommendations have been published ( https://ift.tt/3nxpAyz ). This paper is a summary of the key recommendations including the following: start early, preferably in parallel to designing the medicine’s development plan and individual protocols; identify the rationale and clinical need; listen to the perspectives of children and families, and of patient advocacy groups; identify how many peo

Ethical issues in treatment of babies born at 22 weeks of gestation

Many centres now report that more than half of babies born at 22 weeks survive and most survivors are neurocognitively intact. Still, many centres do not offer life-sustaining treatment to babies born this prematurely. Arguments for not offering active treatment reflect concerns about survival rates, rates of neurodevelopmental impairment and cost. In this essay, I examine each of these arguments and find them ethically problematic. I suggest that current data ought to lead to two changes. First, institutional culture should change at institutions that do not offer treatment to babies born at 22 weeks. Second, we need more research to understand best practices for these tiny babies. from Archives of Disease in Childhood current issue https://ift.tt/30AQ09s

Hypnosis and functional neurological disorder

This is a great paper and you must read it! Archivist has always been interested in the interaction between the psychological and the physical when managing patients. Justin Coogle et al (Pediatric Neurology 2021;125:20–5) begin their paper stating that the treatment of functional neurological disorder (FND), previously referred to as conversion disorder, can be challenging. Most general paediatricians, paediatric neurologists and child psychiatrists will have been challenged by such cases and rely heavily on a multidisciplinary team (MDT) to help their patients. Maybe a hypnotist should be part of the MDT. This paper is a case report but is a fantastic review of FND and the role of hypnosis. Jean-Martin Charcot, used therapeutic hypnosis as early as the 19th century and developed the concept of the novel use of a hypno-anaesthetic technique (the magic glove). There is a great section on the history of FND and hypnosis exploring the... from Archives of Disease in Childhood current

Pro-social hormones for autism?

What is a pro-social hormone? Well, apparently the neuropeptide, oxytocin could be one. In the search to find medication to help improve social function in some children with autism, intranasal oxytocin has been used. Does it work? Sikich L et al ( N Engl J Med 2021;385: 1462–1473) have conducted a 24-week, placebo-controlled phase 2 trial of oxytocin therapy in children and young adults, 3 to 17 years of age with autism spectrum disorder. Out of 355 subjects screened, they enrolled 290 recruits who were randomly assigned in a 1:1 ratio, with stratification according to age and verbal fluency, to receive oxytocin or placebo. The oxytocin was administered intranasally at a total target dose of 48 international units daily. The primary outcome was the least-squares mean change from baseline on the Aberrant Behaviour Checklist modified Social Withdrawal subscale (ABC-mSW), which includes 13 items. Secondary outcomes included two additional measures of... from Archives of Disease in Ch

Use of rituximab in paediatric nephrology

Rituximab is a chimeric monoclonal antibody capable of depleting B cell populations by targeting the CD20 antigen expressed on the cell surface. Its use in oncology, initially in B cell lymphoma and post-transplant lymphoproliferative disorders, predates its current utility in various fields of medicine wherein it has become one of the safest and most effective antibody-based therapies. It was subsequently found to be effective for rheumatological conditions such as rheumatoid arthritis and antineutrophil cytoplasmic antibody-associated vasculitis. Over the past decade, rituximab has generated a lot of interest in nephrology and has become an emerging or accepted therapy for multiple renal conditions, including systemic lupus erythematosus, lupus nephritis, vasculitis, nephrotic syndrome and in different scenarios before and after kidney transplantation. How to manage protracted bacterial bronchitis

Chronic wet cough or dry cough in children – an important difference in your history taking. Protracted bacterial bronchitis (PBB) is a leading cause of chronic wet cough in children. Current standard treatment is 2 weeks of antibiotics, but the optimal duration of therapy is unknown. Ruffles TJC et al ( Lancet Respir Med 2021;9:1121–29) have described the first randomised controlled trial to assess the duration of antibiotic treatment in children with chronic wet cough and suspected PBB. The editorial by Gilchrist and Carroll [ The Lancet Respiratory Medicine 2021;9:1078–1079) provides an interesting commentary about the importance of the study. They define PBB as a chronic endobronchial infection causing a persistent, troublesome wet cough. Ruffles TJC et al hypothesised that 4 weeks of amoxicillin–clavulanate is superior to 2 weeks for improving clinical outcomes in a parallel, double-blind, placebo-controlled, randomised controlled trial. They have developed a facial deep phenotyping technology based on deep neural networks and facial statistical shape models to screen children for genetic syndromes. They trained the machine learning models on facial photographs from children (aged <21 years) with a clinical or molecular diagnosis of a genetic syndrome and controls without a genetic syndrome matched for..

Does the use of short courses of corticosteroids do any harm?

Paediatricians prescribe short courses of steroids regularly and often the parents will ask if it causes any harm. We then have detailed discussions about long term steroids and the fact that short term courses are unlikely to be of any concern. Yao TC et al (JAMA Pediatr 2021;175:723–29) have asked exactly that: are there potential harms associated with oral corticosteroid bursts (defined as the use of oral corticosteroids for 14 or fewer days) in children? They have quantified the associations of short corticosteroid courses with severe adverse events, including gastrointestinal (GI) bleeding, sepsis, pneumonia, and glaucoma, in children. They have examined 5 years of data from the National Health Insurance Research Database in Taiwan which include 4542623 children. 23% (1064587; 544268 boys [51.1%]; mean (SD) age, 9.7 [5.8] years) were prescribed a single corticosteroid course. Physical restraint for children who have behavioural health related issues

The Archivist has never supported physically restraining angry or violent and emotionally charged children in the emergency department or on the ward. Unfortunately there may be situations where it is felt the only option. Many units will have a physical or chemical restraint guidelines which always causes controversy and most clinicians and nurses, when discussing options with the parents and trying to engage the child, do not find it easy to instigate. Dalton E M et al ( Pediatrics 2021;148:e2020003939) have tackled this issue and provide some excellent advice and suggestions on how to prevent the need for restraint. Children with primary behavioural health conditions are presenting to children’s hospitals with increasing frequency and can be a major challenge in the inpatient environment. Children with severe asthma may be treated with biologic agents normally requiring 2–4 weekly injections in hospital. In March 2020, due to COVID-19, we needed to minimise hospital visits. We assessed whether biologics could be given safely at home. The multidisciplinary team identified children to be considered for home administration. This was virtually observed using a video link, and home spirometry was also performed. Feedback was obtained from carers and young people. Of 23 patients receiving biologics, 16 (70%) families agreed to homecare administration, 14 administered by parents/patients and 2 by a local nursing team. Video calls for omalizumab were observed on 56 occasions, mepolizumab on 19 occasions over 4 months (April–July). Medication was administered inaccurately on 2/75 occasions without any adverse events. Virtually observed home biologic administration in severe asthmatic children, supported by video calls and home spirometry, is feasible, safe and is positively perc

Association between suicide behaviours in children and adolescents and the COVID-19 lockdown in Paris, France: a retrospective observational study

This retrospective observational study conducted in Necker Hospital for Sick Children, France (January 2018–June 2020) evaluated a potential temporal association between admissions for suicide behaviours in children and adolescents and the national COVID-19 lockdown (March–May 2020). During the study period, 234 patients were admitted for suicide behaviours (28% male; mean age 13.4 years). Using Poisson regression, we found a significant decrease in the incidence of admissions for suicide behaviour during the lockdown (adjusted incidence rate ratio: 0.46; 95% CI 0.24 to 0.86). This association might result from reduced help-seeking and decreased hospital admission rates during the lockdown, as well as cognitive and environmental factors. Further multicentre studies should be conducted to confirm these findings and investigate whether a compensatory rise in admissions for suicide behaviour occurred in the postlockdown period. from Archives of Disease in Childhood current issue https:

Keeping young people connected during COVID-19: the role of online groups

The COVID-19 pandemic has had a profound impact on young people, disrupting education, routines, hobbies and peer interactions and there is concern for longer term effects on physical and mental health outcomes. Young people living with chronic health conditions face additional challenges including reduced or no face-to-face contact with medical teams, shielding and the increased stressors of being in ‘at-risk’ groups and social isolation. In a climate of social isolation and disconnectedness, online groups could provide a method of delivering healthcare and support that strengthens social connectedness and reduces isolation. Despite the technology being available, uptake and evidence for online groups is limited. This article shares learnings from a paediatric and adolescent psychology service delivering online groups for young people with chronic health conditions and their healthcare teams. Ideas for how to transfer group process to online platforms are considered, with examples and

Plant based diets

It used to be hard to find a vegan restaurant; now there are many and various expert committees around the world are recommending plant-based diets (PBDs) for both human and planetary health. More people are adopting meat-free diets in industrialised countries. What are the health effects on children eating plant based diets? Desmond MA et al . Mental health in transgender and gender diverse youth

Transgender and gender diverse (TGD) individuals have a gender identity that differs from their gender assigned at birth. The Archivist realises that the whole area of TGD can be complicated and it is important for clinicians to understand some of the key medical and mental health issues as the associated social, political and legal arenas evolve. TGD adolescents (aged 13 to 17 years) are estimated to make up approximately 0.7% of the US population. There is evidence that TGD individuals are at increased risk of mental health problems relative to cisgender (the gender you identify with is the same as the gender you were assigned at birth) peers. Why are certain age bands used for children in paediatric studies of medicines?

Rational prescribing of medicines requires evidence from clinical trials on efficacy, safety and the dose to be prescribed, based on clinical trials. Regulatory authorities assess these data and information is included in the approved summary of product characteristics. Regulatory guidelines on clinical investigation of medicinal products in the paediatric population generally propose that studies are done in defined age groups but advise that any classification of the paediatric population into age categories is to some extent arbitrary or that the age groups are intended only as a guide. The pharmaceutical companies tend to plan their studies using age groups the regulatory guidelines suggest, to avoid problems when applying for marketing authorisation. These age bands end up in the paediatric label, and consequently into national paediatric formularies. The age bands of the most commonly used age-subsets: neonates, infant/toddlers, children and adolescents, are more historical than

Fetal alcohol spectrum disorders: an overview of current evidence and activities in the UK

Estimates for the UK suggest that alcohol consumption during pregnancy and prevalence of fetal alcohol spectrum disorder (FASD)—the most common neurodevelopmental condition—are high. Considering the significant health and social impacts of FASD, there is a public health imperative to prioritise prevention, interventions and support. In this article, we outline the current state of play regarding FASD knowledge and research in the UK, which is characterised by a lack of evidence, a lack of dedicated funding and services, and consequently little policy formulation and strategic direction. We highlight progress made to date, as well as current knowledge and service gaps to propose a way forward for UK research. from Archives of Disease in Childhood current issue https://ift.tt/3gIfIx5

Case for change: a standardised inpatient paediatric early warning system in England

Most children in hospital who are clinically deteriorating are monitored regularly, and their treatment is escalated effectively. However a small, but significant, number of deteriorating children experience suboptimal outcomes because of a failure to recognise and respond to acute deterioration early enough leading to unintended harm. Tragically this occasionally can have fatal consequences. Investigations into these rare events highlight common themes of missed early signs of deterioration in children, prompting regulatory agencies to suggest paediatric early warning systems (PEWS) to aid clinical practice. In England, track and trigger tools (TTT), which are one facet of PEWS have been widely rolled out but in a heterogeneous fashion. The evidence for TTT is mixed but they are complex interventions and current outcomes do not fully define the entirety of their potential impact. This article explains the rationale behind the decision of the NHS England and NHS Improvement, Royal Coll

Metabolic and bariatric surgery

As far as the Archivist goes, bariatric surgery in children and young adults to ‘treat’ extreme weight, remains controversial; especially if completed before puberty and final adult height has been achieved. In the adult world, it appears that metabolic and bariatric surgery (MBS) is an effective, acceptable treatment of severe obesity and is a recommended intervention on most adult guidelines. What about the risks and benefits in young teenagers compared with older adolescents? Ogle SB et al [ Pediatrics 2021;147:e2020024182; DOI: https://ift.tt/3cQTEza ] have reported a non controlled, non randomised, observational study in USA, comparing weight loss, comorbidity resolution, nutritional abstract abnormalities, and quality of life between younger and older adolescents after MBS. From March 2007 to December 2011, 242, young adults who underwent bariatric surgery at these five centres were included in this prospective, multicentre, long-term outcome study; Teen–Longitudinal Assessmen

Lumbar puncture and urinary tract infection in infants

You are faced with a febrile, unwell infant (greater than 29 days and younger than 60 days), and you have identified good evidence that they have a urinary tract infection based urinalysis and microscopy of a supra-pubic sample of urine. Should you complete the infection screen with a lumbar puncture (LP) to rule out a co-existing bacterial meningitis? If they are unwell, most clinicians will complete with LP before starting antibiotics. What about well looking infants with fever and evidence of a urinary tract infection, should they have an LP to rule out meningitis? Burstein B et al [ JAMA Netw Open 2021;4:e214544. doi:10.1001/jamanetworkopen.2021.4544] have examined the most recent evidence in a systematic review and meta analysis. Most published guidelines for febrile young infants recommend lumbar puncture and cerebrospinal fluid (CSF) testing on the basis of a positive urinalysis result to exclude bacterial meningitis as a cause. Tracheostomy trends in paediatric intensive care

Paediatric tracheostomy is most commonly performed in children on the paediatric intensive care unit (PICU) to facilitate long-term ventilation. We sought to identify trends in UK tracheostomy practice in PICUs. Data were analysed from 250 261 admissions, including 4409 children tracheostomised between 2003 and 2017. The incidence of tracheostomy in 2017 was approximately half that in 2003 (incidence rate ratio=0.48, 95% CI 0.40 to 0.57). The percentage of patients tracheostomised during a PICU admission, as a proportion of all admissions, was 2.44% (n=319) in 2003 and reduced to 0.97% (n=180) in 2017. Nevertheless, we identified great variability in practice between different PICUs with tracheostomy rates between 0.0% and 4.0% of all admissions. Risk-adjusted PICU mortality was comparable between tracheostomised children and all admissions to PICU. from Archives of Disease in Childhood current issue https://ift.tt/2SbGjKS
