Early childhood social-developmental interventions

Do trials of preventive social-developmental interventions in childhood, implemented in their early childhood years, have positive outcomes on behavioural and health outcomes into adulthood? Are these changes sustained years after the end of the intervention and do these benefits continue to have an impact across generations? In a non-randomised, controlled, longitudinal interventional study, Hill GK et al [ JAMA Pediatr 2020;174(8):764–771.doi:10.1001/jamapediatrics.2020.1310] examined outcomes on the offspring of individuals who participated in the Raising Healthy Children preventive intervention as children in the 1980s. These subjects were now adults with their own children. This fascinating study showed significant differences in the offspring of individuals who had received the intervention in childhood who were observed across four domains: improved early-childhood developmental functioning, lower teacher-rated behavioural problems, higher teacher-rated academic skills and per

Please use professional interpreters for children and families with limited English proficiency

It is very tempting to use family members as an interpreter when faced with a family that does not speak English. It is sometimes felt inconvenient to wait for an interpreter or having to use a phone interpretation service so attempts are made to communicate with the family. The Archivist feels that this is not acceptable and family members should not be used to discuss sensitive medical issues. Proper informed discussion is required and spending time to make sure you have an interpreter available is essential, even in an emergency. Brophy-Williams S et al ( J Paediatr Child Health 2020;56:1201–1209) have highlighted how common this problem is. They have reviewed the hospital-wide use of interpreters for low English proficiency in a tertiary hospital examining emergency (ED), outpatient and inpatient presentations. They addressed two cohorts of families with low English proficiency presenting to Princess Margaret Hospital in Western Australia; new Refugee... from Archives of Disea

Prescribing emergency oral steroids in asthma clinics

We retrospectively reviewed children who had been prescribed emergency oral corticosteroids (OCS) in a routine tertiary paediatric respiratory clinic appointment. We subsequently assessed adherence from prescription uptake of inhaled corticosteroids or combination inhalers in the 6 months prior to the episode. In 2 years, 25 children received 32 courses of prednisolone. Median adherence was 33%, but 28% for those with repeated OCS prescriptions. Prescribing acute OCS in a routine clinic is a red flag for potential poor adherence to preventer therapies, and may also indicate the child has poor perception of the severity of their symptoms. An assessment of adherence should be carried out and help given to the child and their family to improve poor adherence when detected. from Archives of Disease in Childhood current issue https://ift.tt/2RFrwnP

Variation in the management of Kawasaki disease

Intravenous immunoglobulin (IVIG) reduces coronary aneurysms in patients with Kawasaki disease (KD), but additional management options remain challenging, with no generalisable evidence-based recommendations. We performed a survey of 724 physicians from 73 countries to assess variation in practice. IVIG was the preferred initial treatment by 659 (91%) of respondents. Criteria for adjunctive primary treatment varied considerably and definitions of IVIG resistance varied markedly by geographical continent, Human Development Index tiers and medical specialty. A second dose of IVIG was used most often for patients with coronary aneurysm non-responsive to initial treatment (572, 79%), but corticosteroids (379, 52%) and tumour necrosis factor alpha inhibitors (208, 29%) were also frequently used. Our findings highlight the need for international collaborative efforts to optimise management of patients with KD worldwide. from Archives of Disease in Childhood current issue https://ift.tt/35
