Chronic fatigue syndrome: help young people regain their lives

Twenty years ago the Archivist sat in a clinic for children and young adults with myalgic encephalomyelitis / chronic fatigue syndrome (ME/CFS) with Dr Kathy Rowe. This clinic has been running for well over 30 years. When this clinic started, there was many an eminent educationalist or healthcare clinician who would argue that this diagnosis didn’t exist. It was refreshing to see the results of a long term follow-up study assessing the value of these clinics and the patients cared for within the clinic. Rowe K [ Acta Paediatrica 2020;109:790–800. DOI: 10.1111/apa.15054] reports a prospective cohort study of 784 young people with ME/CFS. These patients provided feedback across a 14-year period regarding helpful strategies and ways to improve their management, using a qualitative questionnaire feedback. Between 1991 and 2009, these children and young adults age 6-18 years, were diagnosed with ME/CFS following referral to a specialised clinic at the Royal... from Archives of Disease i

Public Health England should stop promoting electronic cigarettes and vaping

Public Health England (PHE) must stop promoting vaping as an alternative option to cigarettes, suggesting that is 95% safer than smoking, and instead respond to the actual health emergency in young people. Vaping and smoking are both much more dangerous than abstinence, which should be the message from all responsible bodies. This is the key message in a recent paper from a number of eminent paediatric respiratory physicians based in UK. Bush A et al [ Lancet Resp Med 2020;8(3):231–233. https://ift.tt/34Lzhyf (19)30468-0] have reviewed the recent evidence surrounding the use of electronic cigarettes and the practice of vaping. The Archivist was shocked to read some recent data from the United States of America published in 2019 which found that 40·5% of 12th-grade students (17–18 years) have tried e-cigarettes, 35·1% within the past year and 25·4% in the previous month. The corresponding figures for 2017 are 15·0%, 18·8%, and 11·0%.... from Archives of Disease in Childhood current

Wilson disease in children and adolescents

Wilson disease (WD) is a rare, recessively inherited disorder of copper metabolism mainly affecting liver and brain. In childhood, it is known to have a predominant hepatic phenotype. It is likely that the low awareness for WD-associated neuropsychiatric signs and symptoms in this age group means that neurological Wilson’s disease is underdiagnosed in children and young people. Practitioners should be alert for this complication in children with or without liver disease. Management of children with WD requires a dedicated multidisciplinary approach involving hepatologists, geneticists, neurologists and psychiatrists to ensure subtle neuropsychiatric symptoms are identified early and addressed appropriately. This review highlights recent advances in hepatic and neuropsychiatric symptoms of WD in childhood, specific diagnostic tools and pitfalls and summarises existing and potential future treatment options. from Archives of Disease in Childhood current issue https://ift.tt/2yiSUS5

What does 'PEWing or 'PEWsing mean? A new word in paediatrics

Since 2015, a national Paediatric Early Warning System (PEWS) has been used in acute paediatric settings in the Ireland. At a national PEWS workshop, the terms ‘pewing’ and ‘pewsing’ were noted to have been used or overheard in clinical conversations in some settings. We sought to understand the term ‘pewing’ or ‘pewsing’ interviewing 40 frontline paediatric nurses and doctors in a tertiary hospital and 20 from other regional sites. We asked, ‘what does the term ‘pewing’ or ‘pewsing’ mean to you?’ 95% (38/40) of staff asked at Children Hospital Ireland at Tallaght Hospital heard or used the term daily. No responses indicated the same understanding of the term. Nurses typically reported the term as indicating an abnormal observation, but with no consensus as to what the abnormality was; interpretations varied between PEWS scores of 1 and 7. Doctors echoed this, citing examples such as a ‘deterioration of a patient’... from Archives of Disease in Childhood current issue https://ift.tt/
