As the global epidemic of obesity and type two diabetes in children and adults continues to be a major challenge to public health, an intriguing possible ‘off-target’ effect of the rotavirus vaccine and association with a reduction in the incidence of Type 1 diabetes, has been reported in JAMA Paediatrics . A 14-year-old girl was found moribund in her bed late one morning. She had suffered  from tiredness, headache, nausea and intermittent vomiting during the preceding 4 days, but had been able to continue with normal activities and had received simple analgesics only. She could not sleep well during the two nights before her death and was heard coughing 2 hours before being found moribund by her mother. No signs of life were detected by paramedics, despite prolonged resuscitative attempts. She had attended a general practitioner for a brief period of headache and vomiting 3 months earlier; this was thought to be migraine and was treated effectively with simple analgesia. She remained asymptomatic with no further contact with any health professional until her death. Hello, my name is George, I am 18 years old and have Down’s syndrome. When I was born I had a hole in my heart, and I had to have an operation at 6 weeks old. It was a success and from that time I have really loved my life. A lot of different people have supported me, from the cardiologist all the way through to the genetics team. Other people that have helped include audiologists, podiatrists, orthopaedic surgeons, ENT (ear, nose and throat), ophthalmology, dentistry, speech therapy and occupational therapy. The paediatrics team have joined everything up and without them things would have been complicated. What has worked well generally is that I have had someone who knows me, someone who has worked with me in a team and a person who has supported and helped me. I do have some strengths. Mind the (support) gap: supporting academic trainees through peer networks

Academic Foundation Programme (AFP) trainees face challenges in making the most of the research opportunities available to them. Improved support for trainees throughout the AFP may result in improved outcomes and increase retention of clinical academic trainees. Peer support may address some of the challenges faced by AFP trainees at this early stage of their careers. A Plan-Do-Study-Act (PDSA) approach was used to design and implement a peer support programme for AFP trainees in paediatrics. Feedback from recent AFP graduates was used to identify challenges and possible solutions, which were then incorporated into the programme. Trainee views informed the format of the support offered, especially the role of trainee coordinator and importance of regular meetings. The PDSA approach facilitated constant re-evaluation of the programme, which continued to evolve over several years.
