Conduct of clinical trials in babies, children and young people is often hindered by issues that could have been foreseen before the trial opened; that is, some clinical trials are often underprepared. In order to identify a good approach to trial preparedness, the European Network of Paediatric Research at the European Medicines Agency formed a working group. The Working Group included representation from regulators, industry, academics, paediatric clinical research networks and parents. Ethical issues in treatment of babies born at 22 weeks of gestation

Many centres now report that more than half of babies born at 22 weeks survive and most survivors are neurocognitively intact. Still, many centres do not offer life-sustaining treatment to babies born this prematurely. Arguments for not offering active treatment reflect concerns about survival rates, rates of neurodevelopmental impairment and cost. In this essay, I examine each of these arguments and find them ethically problematic. I suggest that current data ought to lead to two changes. First, institutional culture should change at institutions that do not offer treatment to babies born at 22 weeks. This is a great paper and you must read it! Archivist has always been interested in the interaction between the psychological and the physical when managing patients. Justin Coogle et al (Pediatric Neurology 2021;125:20–5) begin their paper stating that the treatment of functional neurological disorder (FND), previously referred to as conversion disorder, can be challenging. Most general paediatricians, paediatric neurologists and child psychiatrists will have been challenged by such cases and rely heavily on a multidisciplinary team (MDT) to help their patients. Maybe a hypnotist should be part of the MDT. This paper is a case report but is a fantastic review of FND and the role of hypnosis. In the search to find medication to help improve social function in some children with autism, intranasal oxytocin has been used. Does it work? Sikich L et al ( N Engl J Med 2021;385: 1462–1473) have conducted a 24-week, placebo-controlled phase 2 trial of oxytocin therapy in children and young adults, 3 to 17 years of age with autism spectrum disorder. Out of 355 subjects screened, they enrolled 290 recruits who were randomly assigned in a 1:1 ratio, with stratification according to age and verbal fluency, to receive oxytocin or placebo. The oxytocin was administered intranasally at a total target dose of 48 international units daily.
