Does the use of short courses of corticosteroids do any harm?

Paediatricians prescribe short courses of steroids regularly and often the parents will ask if it causes any harm. We then have detailed discussions about long term steroids and the fact that short term courses are unlikely to be of any concern. Yao TC et al (JAMA Pediatr 2021;175:723–29) have asked exactly that: are there potential harms associated with oral corticosteroid bursts (defined as the use of oral corticosteroids for 14 or fewer days) in children? They have quantified the associations of short corticosteroid courses with severe adverse events, including gastrointestinal (GI) bleeding, sepsis, pneumonia, and glaucoma, in children. The Archivist has never supported physically restraining angry or violent and emotionally charged children in the emergency department or on the ward. Unfortunately there may be situations where it is felt the only option. Many units will have a physical or chemical restraint guidelines which always causes controversy and most clinicians and nurses, when discussing options with the parents and trying to engage the child, do not find it easy to instigate. Paediatric severe asthma biologics service: from hospital to home

Children with severe asthma may be treated with biologic agents normally requiring 2–4 weekly injections in hospital. In March 2020, due to COVID-19, we needed to minimise hospital visits. We assessed whether biologics could be given safely at home. The multidisciplinary team identified children to be considered for home administration. This was virtually observed using a video link, and home spirometry was also performed. Feedback was obtained from carers and young people. Of 23 patients receiving biologics, 16 (70%) families agreed to homecare administration, 14 administered by parents/patients and 2 by a local nursing team. Video calls for omalizumab were observed on 56 occasions, mepolizumab on 19 occasions over 4 months (April–July). Medication was administered inaccurately on 2/75 occasions without any adverse events. Association between suicide behaviours in children and adolescents and the COVID-19 lockdown in Paris, France: a retrospective observational study

This retrospective observational study conducted in Necker Hospital for Sick Children, France (January 2018–June 2020) evaluated a potential temporal association between admissions for suicide behaviours in children and adolescents and the national COVID-19 lockdown (March–May 2020). During the study period, 234 patients were admitted for suicide behaviours (28% male; mean age 13.4 years). Using Poisson regression, we found a significant decrease in the incidence of admissions for suicide behaviour during the lockdown (adjusted incidence rate ratio: 0.46; 95% CI 0.24 to 0.86). This association might result from reduced help-seeking and decreased hospital admission rates during the lockdown, as well as cognitive and environmental factors. Further multicentre studies should be conducted to confirm these findings and investigate whether a compensatory rise in admissions for suicide behaviour occurred in the postlockdown period. from Archives of Disease in Childhood current issue https:
