Household economic burden of childhood severe pneumonia in Bangladesh: a cost-of-illness study

Rare case of spontaneous simultaneous extensive subcutaneous emphysema, bilateral pneumothoraces, pneumomediastinum and pneumorrhachis

A 12-year-old boy who was diagnosed with precursor B-cell acute lymphoblastic leukaemia developed a cough and coryza on day 8 of induction. Subsequent throat swab revealed infection with respiratory syncytial virus. He remained well and proceeded with the planned lumbar puncture and administration of intrathecal methotrexate under general anaesthetic. He represented with sudden onset of severe chest and upper back pain radiating to his neck with associated nuchal rigidity. His cough had persisted and intermittently aggravated the pain. Extensive crepitus was felt on palpation of his neck, both shoulders and anterior chest. His heart sounds were reduced anteriorly and had no significant neurological signs or symptoms. Infantile haemangiomas and beta blockers

It was pure serendipity that the French cardiologist recognised that beta-blockers seemed to have an effect on large infantile haemangiomas. The propranolol, which was commenced for the infant’s heart failure also seemed to have an effect on the infant’s haemangioma. Since then, beta-blockers have been considered to the first line treatment for large haemangiomas. Interestingly it took some time before the randomised controlled studies were completed to support the practice and the adverse events became clearer. Choosing which lesion requires treatment is the key decision, as many lesions involute on their own without intervention. Understanding the side effect profile of recommended medication is essential to make a shared management decision with the family. The side effects need to be fully understood. Ji Yi et al [ JAMA Otolaryngol Head Neck Surg . doi:10.1001/jamaoto.2021.0454] have examined two beta blockers in a randomised clinical trial of 377 patients with infantile hemangiom

Childhood bruising

All clinicians caring for children will be challenged when faced with a young child covered with bruises. Ruling out leukaemia and idiopathic thrombocytopenia is relatively straight-forward with a blood count and film. Making a judgement about child abuse on the basis of the pattern of bruising is a core skill for every paediatrician; this is sometimes very hard. So can bruising characteristics distinguish abusive from non-abusive injury in young children? Pierce MC et al [ JAMA Netw Open 2021;4:e215832. doi:10.1001/jamanetworkopen.2021.5832] have tackled this question and further developed a clinical decision rule. Over the last 20 years there have been a number of papers and rules published from different groups. This work is further validation of a previously derived bruising clinical decision rule (BCDR), the TEN-4 (bruising to torso, ear, or neck or any bruising on an infant <4.99 months of age. In a cross-sectional study of 2161 children younger than... from Archives of Di

Water fluoridation: current challenges

Dental extraction for caries is the most common reason for children aged 6–10 years to be admitted to hospital. Community Water Fluoridation (CWF) is safe and effective at reducing dental caries. It is most effective where there is more deprivation. However, many deprived areas do not have CWF despite Public Health England recommending it. Those who lobby against fluoridation do so using emotionally charged language and misinformation. We discuss the benefits of fluoridation and the specious arguments used against this important public health measure. The National CWF Network is led by dentists and promotes CWF. COVID-19 has led to the suspension of routine dentistry, renewing the urgency of implementation of CWF. Professional bodies such as the Royal College of Paediatrics and Child Health are urged to give their support. from Archives of Disease in Childhood current issue https://ift.tt/3yl45nL
