Multidisciplinary community paediatric video appointments during COVID-19 pandemic: descriptive study

In responsive to the COVID-19 pandemic, paediatric services adapted working practices. While changes to traditional face-to-face appointments were necessary enabling consultations to continue, it was initially unclear whether new technologies would be reliable or acceptable to staff and patients. Following social distancing commencing in late March 2020, Seaside View Child Development Centre, Brighton, UK (specialising in children with special needs, primarily neurodevelopmental and complex disabilities), converted all appointments to video or telephone formats. The department sought service user and staff experiences of video consultations to assess the impact of this intervention. All families who had AccuRx video consultations, within a 5-week period, were sent anonymised qualitative questionnaires. The medium vessel vasculitis with a predilection for coronary arteries continues to be the leading cause of acquired heart disease in developed countries and is being increasingly recognised in less resourced areas. While the cause remains unclear, epidemiological data suggest an infectious trigger for genetically susceptible individuals. Contemporary treatments have drastically improved outcomes, although a select group of patients continue to experience morbidity beyond the inflammatory phase of the condition. Robust evidence supports the use of intravenous immunoglobulin (IVIG) and aspirin for reducing the risk of KD-related coronary artery aneurysms (CAA). The national implementation groups of early warning systems in the UK and Ireland have identified a need to understand implementation, adoption and maintenance of these complex interventions. The literature on how to implement, scale, spread and sustain these systems is sparse. We describe a successful adoption and maintenance over 10 years of a paediatric early warning system as a sociotechnical intervention using the Nonadoption, Abandonment, Challenges to the Scale-Up, Spread, and Sustainability Framework for Health and Care Technologies. Principles of immunisation in children with solid organ transplant

Vaccine-preventable diseases (VPD) are a significant risk to paediatric solid organ transplant (SOT) recipients on lifelong immunosuppressive therapy. Children progressing to end-stage organ dysfunction are unable to mount a robust immune response. Hence, it is important to plan vaccination early in the course of disease, especially if a child is anticipated to be a SOT candidate. Vaccine recommendations need to be individualised in this population based on vaccine history and serology. Catch-up or accelerated schedules may be used to complete vaccinations before transplant. Post-transplant, immunisation is recommenced in consultation with the transplant team taking into context the time since transplant and the intensity of the immunosuppressive regime. Inactivated vaccines are safe post-transplant but postexposure prophylaxis may still be required in children with inadequate immunity to VPD.
