The UK Turner syndrome (TS) study examined the effect on final height of oxandrolone 0.05 mg/kg/day (maximum dose 2.5 mg) versus placebo from 9 years of age; and delaying ethinylestradiol induction of puberty by 2 years from 12 (E12) to 14 (E14) years in growth hormone-treated girls with TS. The study ran from 1999 to 2013. By 2011, eighty-two of 92 participants had reached final height and an interim analysis using the Super-Imposition by Translation And Rotation model showed significant increases in final height with both oxandrolone and E14. The analysis has been repeated now that all 92 patients have reached final height. Oxandrolone still significantly increased final height by 4.1 cm (95% CI 1.6 to 6.6, n=92) compared with 4.6 cm previously. The epidemiology of meningococcal disease is changing in the UK. It is now a rare cause of non-blanching rashes in children presenting to the emergency department. All paediatricians will have been faced with a child with a non-blanching rash. The dilemma is well rehearsed: should this child have invasive investigations and administration of intravenous antibiotics in order to treat possible meningococcal disease or should they simply be observed for some time or indeed discharged with a clinical diagnosis of probable enterovirus without any intervention. The risk is of course, missing severe sepsis and the possible unnecessary death of a child vs inflicting many more children to unnecessary investigations, over treatment with antibiotics and unnecessary hospital admission and cost and contributing to the more global issue of antimicrobial resistance. Accommodation outcomes for children of drug-misusing women

Children of drug-misusing women are a vulnerable group. Data describing the pattern of accommodation placements are lacking. We investigated 10- to 12-year accommodation outcomes of children born to drug-misusing mothers at a single maternity hospital. 94% of mothers were prescribed maintenance methadone during pregnancy and at least 87% poly-drug used. Data were successfully matched for 132 children (29% of the original cohort of 450 babies). These children had a total of 291 placements (median 2 (range 1–6)), only 28.5% of which were with the birth parents. At 10–12 years, 54 (41%) were in the care of their parent(s). 83% (109/132) were discharged from the maternity hospital to their birth parents; 41% of these children (54/132) remained with their parents at 10–12 years. Better understanding of the proportion of children who ever receive special educational needs (SEN) provision or social care services during school years is highly relevant for healthcare as reductions in one or more of these services could impact on healthcare. Using the National Pupil Database linked to the all-of-England children looked after return and children in need census, we estimated the cumulative incidence of SEN status among (1) children ever in care during school, (2) children in need but not care, and (3) neither. We observed a very high proportion of children who were in care or need during school years had SEN provision at some point (83% and 65%, respectively), and that a high proportion of children in neither of these groups did so, too (37%).
