Above all the practice of medicine is a discipline of direct contact; diagnosis is built on history and examination. In paediatrics, there may only be a secondhand narrative, so the art of recognising illness from observation alone makes it an even more appealing but challenging career. Recent evidence suggests that far more adolescents are unhappy with their biological gender than we ever thought in the past. We also know that transgender adults have disturbingly high levels of mental illness and damaging substance abuse, perhaps because of their perceived torment of being ‘born into the wrong body’. So to what extent do such feelings affect the mental health of teenagers? Coincidentally, two US studies have come to much the same conclusions: that adolescents with gender identity issues have poorer mental health. The first was a secondary analysis of data from the US-wide Profiles of Student Life survey, which gathered questionnaire responses from over 120 000 young people aged 11 to 19 years between 2012 and 2015 (Toomey R et al doi: 10. 1542/peds0.2017–4218). Fatal anaphylaxis: what are the risks?

The UK media recently reported two tragic cases involving death from anaphylaxis, both involving food from a high-street retail chain (bbc.co.uk/news/uk-england-london-45617845; bbc.co.uk/news/uk-45774709). One of these was a child. The publication of a wide-ranging review of fatal anaphylaxis is therefore timely (Pouessel G et al. Clin Exp Allergy 2018. doi: 10.1111/cea.13287). They analysed and compared 32 different studies which reported epidemiological and risk factor data from 9 countries, in children and adults. In spite of public perceptions, anaphylaxis deaths remain rare: less than 1 per million population per year for all causes, of which only 0.03 to 0.3/million/year are food-related. These may be underestimates because of misattribution of cause of death. In most countries the fatality rate has changed little over the past 2 decades, in spite of allergy becoming more prevalent. Patients with cerebral palsy (CP), especially those at the severe end of the spectrum (Gross Motor Function Classification System levels IV–V equivalent), frequently suffer from sleep disturbance and sleep-disordered breathing (SDB). Non-invasive ventilation (NIV) is increasingly used in this patient group, albeit with little published evidence of its effectiveness in CP. This article aims to review the current evidence in the use of NIV in children with CP, highlighting areas of uncertainties, as well as the balance of potential risks, challenges and benefits. We would also share our experience and practical considerations in using NIV to manage SDB in this group of patients. Between January 2010 and December 2016, 21 patients (median age 11.1 (range 1.7–16.1) years) with CP were initiated on NIV at Great Ormond Street Hospital for Children following diagnosis of moderate to severe SDB on cardiorespiratory polygraphy.
