In recent decades, overall child mortality in low-income countries has improved dramatically, but the same rate of improvement has not been seen in neonatal mortality: in South Asia, the proportion of all child deaths that occurred in the neonatal period rose from 45% in 1990 to 59% in 2017. The cause of most neonatal deaths is obvious—congenital anomalies, prematurity or birth asphyxia—but an estimated one-third of deaths appear to be due to infection. To tackle this problem we need to know what organisms are responsible. The difficulty is that in resource-poor settings, there are usually no facilities for microbiological diagnosis. An important study conducted at five sites in deprived South Asian communities (four rural in Bangladesh and India, one urban in Pakistan) recruited over 60 000 pregnant women between 2011 and 2014 (ANISA study. What happens to our adolescent patients when they leave our care? At least in the UK, the suspicion is that the care they receive from adult services is not as good. Most of the evidence for this has come from disease-specific or subjective qualitative studies. A cross-sectional study using universal standard hospital episode statistics from NHS England gives some more objective support for this view (Wijlaars L et al. Currently, the diagnosis of congenital cytomegalovirus (cCMV) infection in most highly resourced countries is based on clinical suspicion alone. This means only a small proportion of cCMV infections are diagnosed. Identification, through either universal or targeted screening of asymptomatic newborns with cCMV, who would previously have gone undiagnosed, would allow for potential early treatment with antiviral therapy, ongoing audiological surveillance and early intervention if sensorineural hearing loss (SNHL) is identified. This paper systematically reviews published papers examining the potential benefits of targeted and universal screening for newborn infants with cCMV. We found that the treatment of these infants with antiviral therapy remains controversial, and clinical trials are currently underway to provide further answers.
