The ‘Back to Sleep’ campaign in the early 1990s dramatically reduced the incidence of sudden infant death syndrome (SIDS), but the problem has not gone away. There have always been familial clusters, some of which have led to parents being convicted of serial infanticide. In these legal cases the defence has been some genetic predisposition to SIDS, but this has been difficult to prove. A popular theory is that SIDS only occurs when three factors are present: a high-risk environment, a critical phase of development and a genetic vulnerability. It therefore seems reasonable to look for gene mutations that increase SIDS risk but cause no other ill-effects. The molecular biology of skeletal muscle channelopathies has now been well-described. Mutations of the gene SCN4A can alter the sodium channel NaV1.4 and thus alter skeletal muscle excitability. Ever since inhaled corticosteroids (ICS) were first introduced as an effective prophylactic agent, there has been a debate over whether asthma exacerbations can be treated by increasing the dose. In theory, as we know systemic steroids are highly effective, they should work. A popular strategy is doubling the dose at the first sign of an exacerbation (sometimes termed the ‘yellow zone’), but trials have not shown this to be effective. Could increasing the dose by a factor of four or five work better? The NEJM recently published two trials side-by-side addressing this question, which came to opposing conclusions. Infant home respiratory monitoring using pulse oximetry

Respiratory rate (RR) is a valuable early marker of illness in vulnerable infants, but current monitoring methods are unsuitable for sustained home use. We have demonstrated accurate measurement of RR from brief recordings of pulse oximeter plethysmogram (pleth) trace in full-term neonates in hospital. This study assessed the feasibility of this method in preterm infants during overnight recordings in the home. We collected simultaneous overnight SpO 2 , pleth and respiratory inductive plethysmography (RIP) on 24 preterm infants in the home. RR from pleth analysis was compared with RR from RIP bands; pleth quality was assessed by the presence of visible artefact. Median (range) RR from RIP and pleth were not significantly different at 42 (25–65) and 42 (25–64) breaths/min. Median (range) % of epochs rejected due to artefact was 20 (8–75) for pleth and 10 (3–53) for RIP. Abusive head trauma and the triad: a critique on behalf of RCPCH of 'Traumatic shaking: the role of the triad in medical investigations of suspected traumatic shaking

The Swedish Agency for Health Technology Assessment and Assessment of Social Services (SBU) has recently published what they purported to be a systematic review of the literature on ‘isolated traumatic shaking’ in infants, concluding that ‘there is limited evidence that the so-called triad (encephalopathy, subdural haemorrhage, retinal haemorrhage) and therefore its components can be associated with traumatic shaking’. This flawed report, from a national body, demands a robust response. The conclusions of the original report have the potential to undermine medico-legal practice. We have conducted a critique of the methodology used in the SBU review and have found it to be flawed, to the extent that children’s lives may be put at risk. Paediatric outcomes and timing of admission

Studies of adult patients have demonstrated that weekend admissions compared with weekday admissions had a significantly higher hospital mortality rate. We have reviewed the literature to determine if the timing of admission, for example, weekend or weekday, influenced mortality and morbidity in children. Seventeen studies reported the effect of timing of admission on mortality, and only four studies demonstrated an increase in those admitted at the weekend. Meta-analysis of the results of 15 of the studies demonstrated there was no significant weekend effect. There was, however, considerable heterogeneity in the studies. There were two large UK studies: one reported an increased mortality only for planned weekend admissions likely explained by planned admissions for complex conditions and the other showed no significant weekend effect.
