In August 2016, Lucina featured two new treatments likely to be highly effective in treating haemophilia (doi101136/archdischild-2016-31162). Now here’s another, even more radical, which promises to actually correct the genetic defect. The X-linked haemophilia gene, which spread to all the royal houses of Europe in the early 20 th century by virtue of Queen Victoria’s fecundity, affected the heir to the Russian throne and was arguably responsible for the 1917 Russian revolution. A successful gene therapy study involving patients with haemophilia A from 5 UK centres has now been published (Rangarajan S et al . NEJM 2017. doi:10.1056/NEJMoa1708483). This follows a successful similar trial for the rarer haemophilia B (doi:10.1056/NEJMoa1708538). Although the subjects were all adult men, its implications for haemophiliac children are obvious. Oxygen is the most common treatment for newborns in need of respiratory support. However, oxygen can cause tissue injury through reactive oxygen species formation, especially in premature infants with reduced antioxidant defences, and may result in short-term and long-term toxic effects in multiple organ systems. Although most hospitals have the capability to tightly control oxygen delivery to hospitalised neonates, in many circumstances, the need is overlooked during infant transport. Lack of awareness of harm or appropriate medical equipment invariably results in excessive oxygen exposure. We usually think of anorexia nervosa (AN) and other eating disorders (ED) as primarily psychological conditions. However there is a surprising amount of evidence suggesting that autoimmunity has something to do with it. Some autoantibodies can have a direct effect on brain neurons causing obvious behavioural disorders, as a seen in PANDAS (paediatric autoimmune neuropsychiatric disorders associated with streptococcal infections): indeed some PANDAS-type cases have been reported to have acute-onset anorexia. Two large population based studies from Scandinavia throw some light on this puzzling association. The first, from Sweden, used their national registers to link people with intestinal biopsies that were unequivocally positive for coeliac disease (CD) with a formal diagnosis of AN (by ICD-8, 9 or 10 criteria), before or after the CD diagnosis (Mårild K et al . Treatment and management of children with haemolytic uraemic syndrome

Haemolytic uraemic syndrome (HUS), comprising microangiopathic haemolytic anaemia, thrombocytopaenia and acute kidney injury, remains the leading cause of paediatric intrinsic acute kidney injury, with peak incidence in children aged under 5 years. HUS most commonly occurs following infection with Shiga toxin-producing Escherichia coli (STEC-HUS). Additionally, HUS can occur as a result of inherited or acquired dysregulation of the alternative complement cascade (atypical HUS or aHUS) and in the setting of invasive pneumococcal infection. The field of HUS has been transformed by the discovery of the central role of complement in aHUS and the dawn of therapeutic complement inhibition. Respiratory manifestations of gastro-oesophageal reflux in children

Gastro-oesophageal reflux disease (GORD) is a complex problem in children. Suspected respiratory manifestations of GORD, such as asthma, chronic cough and laryngitis, are commonly encountered in the paediatric practice, but continue to be entities with more questions than answers. The accuracy of diagnostic tests (ie, pH or pH-impedance monitoring, laryngoscopy, endoscopy) for patients with suspected extraoesophageal manifestations of GORD is suboptimal and therefore whether there is a causal relationship between these conditions remains largely undetermined. An empiric trial of proton pump inhibitors can help individual children with undiagnosed respiratory symptoms and suspicion of GORD, but the response to therapy is unpredictable, and in any case what may be being observed is spontaneous improvement. I have struggled with spelling for most of my life and occasionally I still pronounce words incorrectly. (For about four Harry Potter books, I pronounced Hermione ‘Hermy-one’ in my head, instead of ‘Her-my-oh-knee’, for example.) So every time I see the word ‘incongruous’, I tend to get slight shivers of spelling test related fear. It is, however, a very good work to describe the findings of many folks when they start to look at research in an area. There are a small number of studies that seem to give a strikingly different answer to others... they are incongruous. This finding should lead to examining why there are differences of answer appearing. Is it that one study is overwhelmingly large, and the others represent smaller, publication-biased estimates?
