The development of radically new treatments for cystic fibrosis (CF) proceeds apace. In February 2018 Archivist reported on trials of double-modulator therapies (tezacaftor-ivacaftor) that produced modest benefits (doi:10.1136/archdischild-2017–3 14 602). Adding a third agent could, in theory, further enhance the intracellular functioning of the defective CFTR protein by binding to different sites on the molecule, particularly in those with the most common Phe508del mutation. Two such agents which showed promise in vitro were used in parallel multinational phase two randomised controlled trials, published simultaneously in the NEJM . Both involved adults only, but the implications for children are obvious. Can an 'artificial pancreas improve diabetic control?

In type-one diabetes (T1D), pumps delivering continuous insulin infusions alongside continuous blood glucose monitoring are increasingly becoming standard management. The next logical step is to link these devices intelligently, so that the patient does not have to check their glucose levels at all: known as a ‘closed-loop’ system, it is effectively an electronic ‘artificial pancreas’. But does this advanced technology actually improve diabetic control? Some early small adult-only trials were inconclusive, but a more definitive randomised controlled trial from six centres in the UK and USA is more enlightening (Tauschmann M et al. Lancet 2018. doi.org/10.1016/S0140-6736(18)31947-0). They recruited adults and children over 6 years old with T1D, who were already using insulin pumps but still had poor control. There are some pretty big issues here, not least the idea that studies are undertaken in an unreal world, where troubles melt like chimney tops and sail above your lemon drops. Above all the practice of medicine is a discipline of direct contact; diagnosis is built on history and examination. In paediatrics, there may only be a secondhand narrative, so the art of recognising illness from observation alone makes it an even more appealing but challenging career. Medical education has previously concerned itself with teaching the clinical signs of illness (eg, chest retractions) or any abnormal postures or positions adopted (ie, in a child with a dystonia) or the sounds made (ie, stridor in upper airways obstruction). Recent evidence suggests that far more adolescents are unhappy with their biological gender than we ever thought in the past. We also know that transgender adults have disturbingly high levels of mental illness and damaging substance abuse, perhaps because of their perceived torment of being ‘born into the wrong body’. So to what extent do such feelings affect the mental health of teenagers? Coincidentally, two US studies have come to much the same conclusions: that adolescents with gender identity issues have poorer mental health. The first was a secondary analysis of data from the US-wide Profiles of Student Life survey, which gathered questionnaire responses from over 120 000 young people aged 11 to 19 years between 2012 and 2015 (Toomey R et al doi: 10. 1542/peds0.2017–4218). Fatal anaphylaxis: what are the risks?

The UK media recently reported two tragic cases involving death from anaphylaxis, both involving food from a high-street retail chain (bbc.co.uk/news/uk-england-london-45617845; bbc.co.uk/news/uk-45774709). One of these was a child. The publication of a wide-ranging review of fatal anaphylaxis is therefore timely (Pouessel G et al. Clin Exp Allergy 2018. doi: 10.1111/cea.13287). They analysed and compared 32 different studies which reported epidemiological and risk factor data from 9 countries, in children and adults. In spite of public perceptions, anaphylaxis deaths remain rare: less than 1 per million population per year for all causes, of which only 0.03 to 0.3/million/year are food-related. These may be underestimates because of misattribution of cause of death. In most countries the fatality rate has changed little over the past 2 decades, in spite of allergy becoming more prevalent. Use of non-invasive ventilation in cerebral palsy

Patients with cerebral palsy (CP), especially those at the severe end of the spectrum (Gross Motor Function Classification System levels IV–V equivalent), frequently suffer from sleep disturbance and sleep-disordered breathing (SDB). Non-invasive ventilation (NIV) is increasingly used in this patient group, albeit with little published evidence of its effectiveness in CP. This article aims to review the current evidence in the use of NIV in children with CP, highlighting areas of uncertainties, as well as the balance of potential risks, challenges and benefits. We would also share our experience and practical considerations in using NIV to manage SDB in this group of patients. Between January 2010 and December 2016, 21 patients (median age 11.1 (range 1.7–16.1) years) with CP were initiated on NIV at Great Ormond Street Hospital for Children following diagnosis of moderate to severe SDB on cardiorespiratory polygraphy. Latent or primary tuberculosis (TB) is often diagnosed in vulnerable communities, and to completely eliminate the mycobacterium requires a long course of treatment. The use of long-term ventilation (LTV) in children is growing in the UK and worldwide. This reflects the improvement in technology to provide LTV, the growing number of indications in which it can be successfully delivered and the acceptability of LTV to families and children. In this article, we discuss the various considerations to be made when deciding to initiate or continue LTV, describe the process that should be followed, as decided by a consensus of experienced physicians, and outline the options available for resolution of conflict around LTV decision making. We recognise the uncertainty and hope provided by novel and evolving therapies for potential disease modification. This raises the question of whether LTV should be offered to allow time for a therapy to be trialled, or whether the therapy is so unlikely to be effective, LTV would simply prolong suffering. In recent decades, overall child mortality in low-income countries has improved dramatically, but the same rate of improvement has not been seen in neonatal mortality: in South Asia, the proportion of all child deaths that occurred in the neonatal period rose from 45% in 1990 to 59% in 2017. The cause of most neonatal deaths is obvious—congenital anomalies, prematurity or birth asphyxia—but an estimated one-third of deaths appear to be due to infection. Currently, the diagnosis of congenital cytomegalovirus (cCMV) infection in most highly resourced countries is based on clinical suspicion alone. This means only a small proportion of cCMV infections are diagnosed. Diabetic ketoacidosis (DKA) can be challenging to manage. In the vast majority the outcome is good, but in a few cerebral oedema can develop rapidly and unpredictably, resulting in neurological impairment and, occasionally, death. It is more common in children than adults, and its pathophysiology has traditionally been attributed to rapid changes in blood osmolality causing fluid to shift into brain cells. For these reasons, recommended intravenous rehydration regimes for  children have differed from those for adults, using lower rates and volumes, and isotonic rather than hypotonic solutions. The consequences of vitamin D and dietary calcium deficiency have become a huge public health concern in the UK. The burden of disease from these deficiencies includes rickets, and hypocalcaemic seizures, dilated cardiomyopathy and mostly occult myopathy and osteomalacia. The increasing burden of the disease is intrinsically linked to ethnicity and the population demographic changes in the UK. Newborn babies positively screened for cystic fibrosis (CF) (high serum immunoreactive trypsin (IRT) with DNA analysis) are referred for a diagnostic sweat test, which may be normal (sweat chloride <30 mmol/L). Unless two gene mutations are identified during Newborn screening (NBS), the babies are discharged from follow-up. We wished to check that none had subsequently developed symptoms suggestive of CF. We retrospectively reviewed patient notes and contacted general practitioners of all babies with a negative sweat test, conducted in one of the four paediatric specialist CF centres in London, over the first 6 years of screening in South East England. Of 511 babies referred, 95 (19%) had a normal sweat test. Five (5%) had CF diagnosed genetically, two of them on extended genome sequencing after clinical suspicion. The massive differences in child mortality between low and high-income countries are well-known, but differences between similarly wealthy countries are also striking, and inter-country comparisons can tell us much about the causes. England and Sweden are both prosperous Western European countries with universal social security and healthcare, so one might expect mortality rates to be similar: not so. As more countries adopt sickle cell screening programmes, it becomes essential to have periodic data assessments to determine overall effectiveness and identify areas for improvement.
